The Office of Assessment keeps careful documentation of all requests for data and access privileges. 
 

Opt-out of the Student Accessible Course Evaluation System. 

By vote of the Arts & Sciences Council in Fall 2021, the process for instructors to share evaluation results in the Watermark platform functions by default opt-in. Instructors who choose to rescind student access to results must fully complete a submission of the SACES Selection form after reports are made available and before the deadline, which is set the first day of the month in which “shopping carts” open for the following term. Late requests to opt out will not be approved. No new changes will be made to the old SACES reports in Tableau, which have now been archived. Instructors should use the following form to communicate their wish to opt-out of releasing reports to students. [Qualtrics web form here
 

Access to the Watermark Course Evaluations and Surveys (CES) course evaluation environment

This form is intended for department and program officers and designated staff who require access to course evaluation set-up and reporting in Watermark Course Evaluations and Surveys (CES). The web form requires staff to indicate the name and email of the department Chair or DUS for verification of access eligibility. The Office of Assessment conducts biennial audits of course evaluation access to remove personnel whose roles or responsibilities no longer require report access.  [Qualtrics web form here]
 

Access to Tableau reports

This form is intended for department and program officers and designated staff who require access to departmental reports. Descriptions of the available dashboards can be found here.
